Qualifications:

• For General Practitioners, Ophthalmologists, Dentists, Optometrists, and Veterinarians
• 21-59 years old, Filipino Citizen

Requirements:

• Completely filled up application form
• Photocopy of 2 Gov’t Valid IDs
• Latest Proof of Billing (Electric or Water Bill)
• 2 pcs 2×2 picture

Major Requirements:

• Latest 3 Consecutive Month Bank Statements
• Updated PRC ID
• Latest Proof of Billing (Electric or Water Bill)
• Proof of Income (ANY OF THE FOLLOWING)
  • Business Permits
  • Latest COE/Latest Pay slip/Company ID (if employed)
  • Latest 3 months bank statement

Co-borrower (is required if borrower is RENTING)

• Photocopy of 2 Gov’t Valid IDs
• 2 pcs 2×2 picture
• Marriage Cert if Spouse as co-borrower
• Birth Certificate of child (if live-in as co-borrower)

Other documents may be required as needed.
